  1. Child Abuse History Clearance – There is a cost of $13.00 for this clearance. Please complete this clearance online at PA Child Abuse Clearance www.compass.state.pa.us/cwis to receive your results quickly. Your results will be provided to the email address you used during registration This clearance will need renewed every 5 years. 
  2. Criminal Records Check – There is a cost of $22.00 for this clearance. Please complete this clearance online at PA Criminal Record Check http://epatch.state.pa.us and receive your results quickly. Please print the certificate from the “Certification Form” This clearnace will need renewed every 5 years. 
  3. Federal Criminal History Record Information Clearance (FBI Fingerprints) – There is a cost of $22.60 for this clearance. You must register (FBI Fingerprint Clearance https://www.identogo.com/) to be fingerprinted at one of their locations using Service Code: 1KG6XN. Individuals will be given a receipt with the Registration UE ID that must be given to the Transportation Department to access their report. This clearance will need renewed every 5 years. 
  4. Mandated Reporter Training – This training provides information on the recognition of child maltreatment and the obligation and procedes to report child maltreatment. Mandated Reporter Training http://www.reportabusepa.pitt.edu/ Please print and return Certification of Completion to the Transportation Department. This clearance will need renewed every 5 years. 
  5. Act 24 Arrest and Conviction Form – Act 24 Arrest and Conviction Form.pdf This clearance will need renewed every 5 years. 
  6. TSD Yearly Check List Form – TSD Yearly Check List Form - 2025&2026.pdf This clearance will need renewed every year. 
  7. Physical – Non CDL and CDL Drivers are required to have yearly physical examinations. Below are both physical forms.
